Pre-Kindergarten Application
Application Process
If your child will be four years old by December 31, your child is eligible for Universal Pre-Kindergarten (UPK) services at a public school or contracted community based organization(CBO).
For information about the Pre-Kindergarten Application Process and location of pre-Kindergarten services, please:
- Go to the DOE Web site at
http://schools.nyc.gov/ChoicesEnrollment/PreK; or
- Visit your local elementary school to pick up the pre-Kindergarten Application Packet.
Documents
When registering your child, please bring documnets that verify the child's age, address, and immunization status.
Proof of Age
The child's official birth certificate; or
An official passport showing the date of birth.
Proof of Address
Must be dated within 60 days of presentation.
Any one of the below:
- Residential Utility Bill (electric/gas issued by Keyspan, Con Edison or the Long Island Power Authority)
- Federal, state or local government agency document on letterhead(for example, IRS, City Housing Authority, Administration for Child Services or an ACS subcontractor indicationg the resident's name and address)
- Address Affidavit, if applicable, as per Chancellor's Regulation A-101
OR, any combination of two from the below list:
- Original lease, deed, or mortgage statement
- Property tax bill
- Water bill
- Official payroll documentation (for example, tax form/receipt)
Please note that children must be present when you register for pre-Kindergarten services!
Pre-Kindergarten Health Requirements
Parents are required to ensure that their children meet all health requirements. A New York City Department of Health and Mental Hygiene (DOHMH) physician can conduct an examination if you are not able to obtain one independently.
A physical examination is required of ALL new entering students.
Immunization Requirements
Some immunization requirements for New York City children have been added during the past few years. The Department of Health and Mental Hygiene requires all new entrants to be screened for the following: HIB(pre-K only), DPT, Polio, MMR, HBV, and Varicella. The information below explains the required vaccination schedule:
Children who do not completely meet these requirements may be admitted provisionally with a plan to complete the remaining doses in the vaccination schedule.
